CustomLensAI — Pre-Session Orientation

CustomLensAI is a software system used in the pre-operative planning of cataract and refractive lens exchange surgery. It reads a patient’s biometry and diagnostic reports, applies the operating surgeon’s pre-configured preferences, and produces a surgical prescription for the intraocular lens.

The system sits in the pre-operative pathway after diagnostic measurements are taken and before surgery. It does not calculate intraocular lens power; that value is taken from the biometry device. CustomLensAI integrates measurements with surgeon preferences to arrive at a lens selection for the eye to be operated on.

Why this evaluation?

This is a usability evaluation conducted ahead of the clinical release of CustomLensAI. Its purpose is to confirm that the safety features built into the workflow (checks, prompts, and confirmations) work as intended when a surgeon uses the device on a real case.

The underlying objective is patient safety. We are evaluating how the device performs, not your clinical skill or judgement.

The clinical workflow

CustomLensAI guides a case through six stages, in order.

#StageCovers
1LifestylePatient’s visual needs and expectations
2HistoryOphthalmic and general medical background
3ExaminationClinical examination findings
4DiagnosticsThe patient’s diagnostic reports
5Surgeon previewYour review of the prepared case
6OR readyThe finalised surgical prescription
Stages 1–3 are prepared in advance by the clinical team; you review them. From stage 4 onward, you work through to the prescription.

Biometry data mapping

The diagnostic reports for this evaluation come from an IOLMaster 700 with Total Keratometry (TK) configured. The application displays these values under standard keratometry labels:

Your session

The session runs in a short, paced sequence of approximately 60 to 70 minutes.
– Orientation — about 5 minutes
– The case — 30 to 40 minutes
– Debrief and ratings — about 20 minutes
– Close — about 5 minutes
You work through the case at your own pace. A facilitator observes but will not guide your clinical decisions. If you ask “what should I do here?” or “is this correct?”, the facilitator will ask you to proceed as you would in practice.

Thinking aloud

While you work through the case, please voice your clinical reasoning as you arrive at decisions. This allows us to follow your process more effectively than a summary at the end. There is no expectation of a running commentary; short phrases in your own voice are exactly right.

Clinical decision examples:

– “I am checking the laterality.”
– “I want to look at the biometry again.”
– “I am thinking about the target refraction for this patient.”
– “I am weighing the lens options.”

If you go quiet, the facilitator may gently invite you to share what you are thinking, though you may ignore the invitation and continue working if you prefer.

Consent and recording

The session is video-recorded for internal review only. It is not shared externally. The patient case is fully anonymised. Your facilitator will go through these points with you at the start of the session and ask for your verbal consent.
